Knowledge/Skills/Abilities:
- Bachelor’s degree in Psychology, Human Services or related activity from an approved University training program - requirement
- Eligible for any/all credentialing required by regulatory and funding sources - requirement
- Reliable transportation and able to travel independently - requirement
- Valid driver’s license
- Meet Tennessee insurance standards
- Driving record which meets AHS’s liability insurance guideline
- Ability to work effectively in a treatment setting with emotionally or socially disturbed individuals
- Proficient in Microsoft Office: Word, Excel, Outlook
- Organized with a strong attention to detail; able to multi-task and prioritize work
- Establish and maintain positive working relationships with all clients, AHS employees and community you interact with as an AHS employee.
Key Job Responsibilities:
- Provision of case management services to include:
- Assessment of ongoing mental health and case management needs
- Planning for mental health services for the consumer in conjunction with the multi-disciplinary team
- Assistance with daily living activities as needed (i.e. housing, food, transportation, etc.)
- Making referrals to appropriate mental health resources as needed and assisting consumer to comply with mental health treatment programs
- To provide designated number of monthly contacts
- If assigned to a Grant funded Program he/she will also be required to provide and follow all grant requirements
- Participate in the development of treatment plan, therapy progress, discharge planning, outreach activities and if applicable individual and group therapy
- Make recommendations regarding the treatment of clients
- Provide clinical backup for other Mental Health Specialists
- Comply with policies and procedures as outlined in AHS’s policy and procedure manual
- Other duties may be assigned by supervisor(s) to meet the needs of the consumer and AHS
Shift Availability:
- Full-Time – Minimum of 40 hours per week (hourly)
